Acute heart failure syndromes: current state and framework for future research.

چکیده
Acute heart failure syndromes (AHFS) poses unique diagnostic and management challenges. This syndrome has recently received attention from researchers, clinicians, regulatory agencies, and the pharmaceutical industry. However, there is no consensus on its definition, epidemiology, pathophysiology, appropriate therapeutic options, and directions for future research. This document is the result of the First and Second International Workshop on Acute Heart Failure Syndrome that took place in May 2004 and April 2005. At these workshops, a selected group of physician scientists, epidemiologists, clinicians, regulatory and governmental funding agencies, and industry representatives from North and South America and Europe convened to develop a platform for future investigative approaches and management of AHFS. Subsequently, emergency physicians, who play a pivotal role in the early management of AHFS, contributed to this document.

Therapy for acute heart failure syndromes.
The pharmacologic management of acute heart failure syndromes (AHFS) has changed little over the past 15 years. Traditional therapies, such as nitrates and loop diuretics, remain the mainstay of therapy, with inotropes reserved for patients who present in shock or an advanced low-output state. We review the use of these therapies in AHFS with added insights from recent clinical trials and regis...
متن کاملThe current and future management of acute heart failure syndromes.
Hospitalization for heart failure (HF) marks a substantial crossroad for patients, as greater than one-third will be re-hospitalized or dead within 90 days post-discharge. For patients with chronic HF who present with acute heart failure syndromes (AHFS), they transition from an arena of well-established and life-saving evidence-based therapies to one where early pharmacological management has ...
متن کاملA review of dyspnea in acute heart failure syndromes.
In acute heart failure syndrome (AHFS), dyspnea is one of the most common but least understood presenting symptoms for hospitalization. For this reason, dyspnea relief is increasingly becoming a focus in the development of therapies for the treatment of AHFS, and currently stands as an acceptable primary end point for regulatory approval by governmental agencies. This raises the question of how...
متن کاملA review of phase II acute heart failure syndromes clinical trials.
Hospitalization for acute heart failure syndromes (AHFS) is a growing health care burden. Over 1 million hospitalizations occur annually, with a postdischarge mortality and rehospitalization rate of ~45% by 90 days and a financial cost greater than $20 billion. Attempts to improve outcomes through novel therapies have largely failed, suggesting new approaches are needed. We review phase II stud...
متن کاملI-4: Male Fertility Preservation Options, Current State-of-The ART and Future Implications
Background Male infertility management has made significant progress during the past three decades, especially after the introduction of intracytoplasmic sperm injection (ICSI) in 1992. However, many boys and men still suffer from primary testicular failure due to acquired or genetic causes. New and novel treatments are needed to address these issues. Spermatogenesis originates from spermatogon...
